The Office of Citizenship and Community Standards appreciates your taking time to complete the Service Evaluation Survey. Feedback from students, faculty, staff, and other University stakeholders assists us in evaluating and improving our services to the University community.  The survey is designed to provide anonymous feedback.  You will submit your answers by clicking on the submit button at the bottom of the page.
