Student Conduct Code Referral Form Instructions and Form
Written Referral: The Office of Citizenship and Community Standards asks that you print the form and mail it to our office. This assists us in keeping referral information confidential as required by our institutional FERPA policy and the Student Conduct Code 8.070.1.
Who: The name of the accused student(s)/student organizations(s) is essential for investigating and charging the appropriate individuals/organizations involved in the alleged violation of the Student Conduct Code and required by the Student Conduct Code 8.070.1.
When and Where: The date and location of the incident and alleged violation of the Student Conduct Code assists in corroborating information in the case from multiple sources. Any complaint should be submitted as soon as possible after the misconduct takes place or becomes known, preferably within ten calendar days (Student Conduct Code, 8.070.1).
What, Why, and How: Description of alleged misconduct assists the University Conduct Officer in verifying the facts of the case and the sections(s) of the Student Conduct Code that are alleged to have been violated.
Complainant Name and Signature: The Student Conduct Code 8.070.1 requires that the name of the complainant referring the incident for investigation be provided. A student/student organization alleged to have violated the student conduct code will be informed of the complainant, unless the University decides to serve as the complaintant. (8.070.1).
Complainant Phone Number and Address: The complainant’s phone number and address is required so that the University Conduct Officer may contact the complainant for further information.
Date of Referral: The Student Conduct Code 8.070.5
requires that cases be investigated and disposed of in a timely
manner. Your signature and the date of the referral verify
that you reported an alleged violation of the Student Conduct
Code on a specific date.
